Continuum Health Partners, a nonprofit health system serving New York, and Jersey City, N.J.-based Horizon Health Center, a Federally Qualified Health Center (FQHC), are implementing Westborough, Mass.-based eClinicalWorks’ EMR and practice management (PM) software in an effort to streamline clinical processes and promote patient safety while reducing costs, says the company.

Continuum will make the software available to three groups in its network: physicians that accept Medicaid and meet the New York City Department of Health and Mental Hygiene’s criteria under its Primary Care Information Project; employed physicians that fall outside of that criterion; and volunteer physicians. Continuum Health Partners is comprised of five hospitals: Beth Israel Medical Center, Roosevelt Hospital, St. Luke’s Hospital, Long Island College Hospital and The New York Eye and Ear Infirmary.

Horizon will support three locations in Hudson County with the new software, making it New Jersey’s first FQHC to implement an EMR/PM system. Horizon providers will also have access to eClinicalWorks Enterprise Business Optimizer for enhanced reporting capabilities, according to the company. Horizon Health Center provides healthcare services to more than 12,000 patients and handles more than 40,000 outpatient visits annually.
