In an effort to prevent the re-hospitalization of home health patients, the Visiting Nurse Service of New York (VNSNY), a not-for-profit home healthcare agency, has selected Viterion TeleHealthcare LLC (Tarrytown, N.Y.), a Bayer — Panasonic Company, as the supplier of telemedicine devices.

According to the company, the Viterion 100 is a device that monitors blood pressure, blood oxygen, blood sugar, weight, and temperature to help healthcare providers quickly determine when a home patient is in need of emergency or acute care.

Under the pilot program, Viterion contends patients in most danger of being re-hospitalized will be equipped with the monitoring device, and the results will be automatically sent to VNSNY and examined by a registered nurse who can identify trends before they become potential problems.
