Chronic Care Company Engooden Partners With Two Health Associations

Engooden Health offering chronic care management to Rural Health Association of Tennessee and HAPevolve, a subsidiary of The Hospital and Healthsystem Association of Pennsylvania

Chronic disease management company Engooden Health has announced partnerships with the Rural Health Association of Tennessee and with HAPevolve, a subsidiary of The Hospital and Healthsystem Association of Pennsylvania (HAP).

Boston-based Engooden offers scalable chronic care management (CCM) services and technology for physicians, health systems and payers to identify rising-risk and chronically ill patients.

The Tennessee partnership gives members of the RHA of TN access to technology-enhanced chronic care management (CCM) services between provider visits. Engooden said it will provide association members with the ability to scale personalized interventions for patients living with chronic diseases to help improve their quality of life and health outcomes.

Hometown Health Clinic, an Engooden Health customer and RHA of TN member, specializes in integrated primary care. Hometown Health treats the whole person’s health needs, including acute care and disease prevention through end of life. Part of its mission is to engage providers that live and work within the communities they serve.

"With the help of Engooden, we can reach our patients that are often hard to reach between clinic visits to check on their care, make sure they’re taking medications, following care plans, and determine if they need any additional services,” said Will Merrick, M.D., at Hometown Health Clinic, in a statement. “Engooden’s care navigators are instrumental in helping us gain a better understanding of the challenges our patients face when it comes to managing their chronic conditions. They empower patients with knowledge and resources that help them take control of their health.”

In Pennsylvania, HAP is a membership services organization that represents 235 Pennsylvania hospitals. Engooden is offering HAP members outsourced chronic care management services. Engooden said its proprietary technology surfaces actionable insights to its care navigators, allowing them to focus on the most clinically important updates and intervene with the right patients at the right time.
